Overview

At Chemawa Indian School, our dedicated faculty and staff strive to create a nurturing and rigorous academic environment. Please review the following information carefully to ensure a smooth and successful application process.
Application Process
To apply for admission, complete the application in full and attach all necessary documents. We will not review applications until we receive all required materials. Falsification or withholding of any information will result in nonacceptance or revocation of admission.
Important Dates
April - Applications for new students will be placed on the school website.
May - Approximately 1-2 weeks after school is dismissed, those students that are invited to return will receive a packet of paperwork to submit to save their spot for the following year. This will be the ONLY communication until travel arrangements have been made. Students will not receive an acceptance letter.
June 15 - early admissions for new students - those with complete packets in by end of May will start receiving notifications of early acceptances.
June 28 - returning students need to have their paperwork returned in order to guarantee a reserved spot for the fall
August 10 - students will be notified for final acceptance of those applications sent in and complete by August 1
Admission Review
The Admissions Committee reviews all complete applications. Some students may need to participate in an online or telephone interview with the admissions committee or administration. After the review, we will notify you by mail or phone regarding the status of your application.
Travel Arrangements
For admitted students, we will provide travel to the school from the home address listed in the application. Parents or guardians are responsible for any withdrawals or emergency travel during the school year.

Return completed applications by mail or email. Please do not fax applications, as they may not be readable.

Required Documents
To review your application, we need copies of the following documents:
- Certificate of Indian Blood
- Birth certificate
- Social Security card (for medical records)
- Transcripts from all high schools attended (unless applying for 9th grade, see below)
- Immunization record (most current)
Additional Documents for 9th Grade Applicants
- 8th grade final report card
- 8th-grade promotion or completion record (certificate, letter or note on the report card)
Conditional Documents (if applicable)
- Current IEP for students requiring Special Education services
- Oregon State Sports Physical Form – required if the student is participating in sports
- Copy of medical insurance card (front and back) – if the student is covered by private insurance
- Court documents for legal custody for a parent or legal guardian
- PO reports/recommendations (if on probation)
- Treatment discharge summaries, aftercare and counseling records or program plan
Note: We will not review incomplete applications. Please ensure all forms are filled out completely.
